How to Apply : Candidate should send the application on plain paper alongwith a recent self signed passport size photograph giving full details as in prescribed format and a Bank DD of Rs.40/- in case of SC category candidates of UP only and Rs.100/- in case of OBC & General category candidate in favour of Director, SGPGIMS Lucknow payable at the SBI SGPGIMS Branch Lucknow (code no. 7789).
